       For consistency of official records, parents are requested to use the

    3) Letter of Recommendation from adviser / guidance  counselor / principal
    4) Latest Report Card / Form 138 (with original, for verification purposes)

B. For applicants under the DepED Homestudy Program:

    Photocopy of the following:

    1) Complete Grade / Level Progress Report
    2) Certificate of Enrollment

C. For Non-Filipinos / Filipinos With Dual Citizenship

    1) Statistics Records / Department of Health (if born in foreign countries)
    2) Alien Certificate of Registration (ACR) and / or official receipt of
